Easy Steps to filling out Health Forms
- Use the fillable PDF of the medical form: BSA Medical Form. Print it out & sign it.
- Submit TWO HARD COPIES of completed forms A, B, C, and a copy of both sides of your insurance card.
- Plan ahead and have forms turned in well before your trip.
